Changes made to email address case sensitivity

There have been some changes to the handling of email addresses.
1. Email addresses are now case insensitive.  This means that demo(at)gmail(dot)com is the same as DeMo(at)gMaIL(dot)coM.
2. As a note, usernames and realnames are case sensitive.  This means that Ima Person is not the same as ima person.
3. The FAQ (http://deankeith.org/notifier/faq.html) has been updated to state the case sensitivity of usernames and realnames.
